♦ To qualify for the $1500 bonus, purchaser must be a bona fide educational, government or religious tax-exempt
institution in the U.S. or Puerto Rico, and purchase a GYHD250STL16, GYHD250STL17 or GYHD250CSTNL
package, new, from an authorized JVC PROFESSIONAL PRODUCTS COMPANY DEALER between October 1,
2008 and March 31, 2009.
♦ To receive the bonus from JVC, the purchaser must fill out the form completely and send it along with a dated
copy of the invoice from the dealer, Tax Exemption form, the original bar code serial numbers cut from both the
GY-HD250 camcorder and RM-HP250 Camera Control Unit carton box, to: JVC PROFESSIONAL, 1700 Valley
Road, Wayne, NJ 07470, Attn: GYHD250 Bonus Fulfillment Department - Promo Code 8088.
♦ The completed claim form must be received by JVC no later than April 15, 2009. Shipping location of the bonus
must be within the U.S. or Puerto Rico. Please allow 6 to 8 weeks for processing of the bonus from the time the
claim has been received by JVC Professional Products Company. JVC is not responsible for lost or destroyed
fulfillment documentation prior to JVC’s receipt. JVC reserves the sole right to determine if the purchaser is a
qualified educational, government or religious institution and may ask for additional information or documentation.
♦ OPTION: In lieu of the end user receiving the bonus from JVC, qualified purchasers may receive an instant bonus
directly from the dealer.
